Ampligen + paclitaxel

Ampligen + paclitaxel is a combination therapy that has been investigated in several clinical trials, particularly for cancer treatment. This combination pairs rintatolimod (Ampligen), an immunomodulatory agent, with paclitaxel, a well-established chemotherapy drug. ## Mechanism and Clinical Development Ampligen (rintatolimod) is a mismatched, double-stranded RNA molecule with immunomodulatory and antiviral properties[6]. It functions as a highly selective agonist of TLR-3, which is expressed in various immune cells, epithelial cells, and solid tumors[3]. When combined with paclitaxel, this therapy appears to create a chemokine-modulating (CKM) regimen that may enhance anti-tumor immune responses. In a Phase 1 study for triple-negative breast cancer (TNBC), 9 patients with stage I-III TNBC were treated with paclitaxel 80 mg/m2 IV weekly for 12 weeks and CKM (which included Ampligen) for the first 3 weeks[1]. The CKM regimen consisted of IV Ampligen 200 mg daily and oral celecoxib 200 mg twice daily for days 1-3, along with varying doses of IFN-α2b[1]. This combination was followed by standard dose-dense doxorubicin and cyclophosphamide (AC) and surgery. ## Clinical Outcomes The completed topline results from this Phase 1 study confirmed positive findings that were previously presented at the 2022 Society for Immunotherapy of Cancer Annual Meeting[1]. The combination demonstrated promising clinical activity in an area where significant immune-related toxicities exist with the current standard of care[1]. ## Other Combination Studies Ampligen is also being investigated in combination with other agents: - In recurrent ovarian cancer, rintatolimod plus pembrolizumab and cisplatin showed an objective response rate of 45% and a clinical benefit rate of 55%[3] - In pancreatic cancer, rintatolimod treatment was associated with higher progression-free and overall survival compared to matched controls[4] - A Phase Ib/II trial is investigating Ampligen in combination with AstraZeneca's Imfinzi (durvalumab) for metastatic pancreatic ductal adenocarcinoma[6] The combination of Ampligen with paclitaxel represents an innovative approach to cancer treatment that leverages both traditional chemotherapy and immunomodulation to potentially enhance therapeutic outcomes, particularly in difficult-to-treat cancers like triple-negative breast cancer.
