Overview

Amrinone (also known as inamrinone) is a bipyridine derivative that functions as a selective phosphodiesterase 3 (PDE3) inhibitor. It exerts both positive inotropic and vasodilatory effects by increasing intracellular cyclic adenosine monophosphate (cAMP) levels in cardiac and vascular smooth muscle. Clinically, it is primarily indicated for the short-term management of severe congestive heart failure. Amrinone has been extensively investigated for its ability to reduce pulmonary vascular resistance and improve right ventricular function in patients with pulmonary hypertension, particularly in the context of cardiac surgery, heart transplantation, and mitral stenosis. Due to potential confusion with amiodarone, the official name was changed to inamrinone in many jurisdictions.
