Overview

Anesthetic Dilating Gel (ADG) is a topical ophthalmic combination formulation used primarily for preoperative preparation in cataract surgery and other ocular procedures requiring both pupil dilation (mydriasis) and local anesthesia. The typical formulation contains phenylephrine (a sympathomimetic mydriatic), tropicamide (an antimuscarinic mydriatic), diclofenac (a nonsteroidal anti-inflammatory drug), and lidocaine (an amide-type local anesthetic). The gel achieves rapid and effective pupil dilation while providing corneal anesthesia, with efficacy at least equivalent to traditional regimens using separate drops. Its mechanism of action involves simultaneous blockade of muscarinic receptors in the iris sphincter muscle, stimulation of alpha-adrenergic receptors in the iris dilator muscle, inhibition of prostaglandin synthesis to reduce inflammation, and stabilization of neuronal membranes to prevent pain signal transmission[2][6].
