ANG4043 is an experimental brain-penetrant peptide-antibody conjugate (PAC) developed by Angiochem for the treatment of HER2-positive brain metastases. It consists of a monoclonal antibody targeting the Human Epidermal Growth Factor Receptor 2 (HER2) chemically linked to Angiopep-2, a proprietary 19-amino acid peptide. Angiopep-2 is designed to exploit the receptor-mediated transcytosis pathway by binding to the Low-Density Lipoprotein Receptor-Related Protein 1 (LRP1) on the blood-brain barrier (BBB) endothelial cells, thereby facilitating the delivery of the large antibody into the central nervous system. Once across the BBB, the antibody component of ANG4043 binds to HER2 on tumor cells, inhibiting their proliferation. This approach aims to overcome the limitations of standard anti-HER2 therapies, such as trastuzumab, which have poor CNS penetration due to the restrictive nature of the blood-brain barrier.
