angiotensin-converting enzyme inhibitor + angiotensin II receptor blocker + finerenone is a combination therapeutic regimen being investigated by the Capital Institute of Pediatrics, China, for the treatment of pediatric Henoch-Schönlein purpura nephritis (HSPN) with mild proteinuria. This regimen involves the addition of finerenone, a selective non-steroidal mineralocorticoid receptor antagonist (MRA), to standard-of-care therapy consisting of an angiotensin-converting enzyme (ACE) inhibitor and an angiotensin II receptor blocker (ARB). The combination aims to provide a more complete blockade of the renin-angiotensin-aldosterone system (RAAS), thereby reducing urinary protein levels and mitigating renal inflammation and fibrosis. Finerenone specifically addresses the aldosterone escape phenomenon often seen with long-term ACE inhibitor or ARB use. This triple-combination approach is currently being evaluated in a Phase 4 clinical trial to determine its safety and efficacy in children aged 3 to 18 years.
