Angiotensinamide is a synthetic octapeptide derivative of bovine angiotensin II and acts as a potent vasoconstrictor and cardiac stimulant. It is primarily used for the treatment of severe hypotension unresponsive to traditional pressor agents. The drug increases blood pressure by causing vasoconstriction through direct action on vascular smooth muscle, especially in small caliber arterioles. Angiotensinamide does not have a direct effect on the heart or cause arrhythmias at therapeutic doses. Its effects are short-lived due to rapid enzymatic inactivation in the bloodstream but can be controlled by adjusting infusion rates. Mechanistically, it exerts its action via agonism at the angiotensin II receptor type 1 (AT1), leading to increased peripheral resistance and elevated blood pressure[1][3][4][5].
