Anti-apoE4 nanobodies are single-domain antibody fragments (also known as nanobodies or VHHs) engineered to recognize and bind specifically to the apolipoprotein E4 (ApoE4) isoform. ApoE4 is a major genetic risk factor for late-onset Alzheimer's disease, as it promotes the aggregation of beta-amyloid (Aβ) plaques and tau pathology in the brain. Nanobodies are much smaller than conventional antibodies and can access cryptic and concave epitopes that are not accessible to full-sized antibodies. Though most published preclinical work targets ApoE4 with full-sized antibodies (e.g., HAE-4), nanobody-based approaches are under research and may act by binding ApoE4, inhibiting its pathological interactions (such as with Aβ), and possibly promoting its clearance or reducing its neurotoxicity. As of now, there is no evidence of clinical-stage development for anti-apoE4 nanobodies, but their use is being explored in preclinical contexts related to Alzheimer's disease and neurodegeneration. Nanobodies, due to their biophysical properties, offer potential advantages such as better brain penetration and reduced immunogenicity compared to conventional antibodies[3][9].
