Overview

Anti-B-cell maturation antigen (BCMA) chimeric antigen receptor (CAR) T-cell therapy is a modality of adoptive cell immunotherapy in which a patient's autologous T cells are genetically engineered to express a CAR directed against BCMA. BCMA is a cell surface protein predominantly expressed on normal and malignant plasma cells, making it an ideal target for treating plasma cell dyscrasias. The therapy involves leukapheresis, ex vivo T-cell expansion and viral transduction (typically using lentiviral or retroviral vectors), and subsequent re-infusion into the patient following lymphodepleting chemotherapy. This approach has demonstrated significant clinical efficacy in patients with relapsed or refractory multiple myeloma, leading to the approval of agents such as idecabtagene vicleucel and ciltacabtagene autoleucel. Key safety concerns include cytokine release syndrome (CRS), neurotoxicity (ICANS), and prolonged cytopenias.
