An anti-BCMA bispecific T-cell engager (TCE) is a therapeutic bispecific antibody designed to treat multiple myeloma by recruiting and activating the patient's own T cells against malignant plasma cells. These molecules feature a dual-binding architecture: one domain targets the B-cell maturation antigen (BCMA), which is highly expressed on the surface of myeloma cells, while the other domain binds to the CD3 epsilon subunit of the T-cell receptor complex. By physically bridging the T cell and the tumor cell, the TCE induces the formation of an immunological synapse, triggering T-cell activation, the release of cytotoxic perforins and granzymes, and the subsequent lysis of the BCMA-positive tumor cell. This modality has shown significant efficacy in patients with relapsed or refractory multiple myeloma, with several agents such as teclistamab and elranatamab receiving regulatory approval.
