Overview

Anti-BCMA CAR T cells with granzyme B-NOXA fusion protein are a next-generation, genetically engineered chimeric antigen receptor T cell therapy designed to treat multiple myeloma. These CAR T cells specifically target the B cell maturation antigen (BCMA) on malignant plasma cells. They are further engineered to express a fusion protein that combines granzyme B, a natural cytotoxic effector molecule, with NOXA, a pro-apoptotic BCL-2 family protein. Upon recognition and contact with BCMA-expressing myeloma cells, the CAR T cells deliver the granzyme B-NOXA fusion protein into the cancer cells through the immune synapse. NOXA localizes to cytotoxic granules and is specifically released into tumor cells, where it antagonizes the anti-apoptotic factor MCL-1 and drives apoptosis, thereby enhancing the killing efficacy of the CAR T cells and overcoming cellular resistance mechanisms. This approach is being developed to address primary resistance and relapse in multiple myeloma patients by boosting tumor cell apoptosis and shows improved efficacy in preclinical models[1][3].
