Anti-BCMA T-cell engagers are a class of bispecific immunotherapies designed to treat multiple myeloma by redirecting the patient's own T cells to eliminate malignant plasma cells. These molecules typically feature two binding domains: one specific for the B-cell maturation antigen (BCMA), which is highly and selectively expressed on the surface of plasma cells, and another specific for the CD3 receptor on T cells. By cross-linking these two cell types, the engager induces T-cell activation and the release of cytotoxic proteins like perforin and granzymes, leading to the lysis of the BCMA-positive tumor cells. This approach provides a potent, off-the-shelf therapeutic option for patients with relapsed or refractory multiple myeloma, with several agents in this class having achieved regulatory approval.
