Anti-BCMA T-cell engagers (TCEs) are a class of bispecific antibodies designed to treat multiple myeloma by redirecting T cells to attack plasma cells. These molecules typically possess two binding domains: one specific for B-cell maturation antigen (BCMA), which is highly expressed on the surface of malignant plasma cells, and another specific for CD3 on the surface of T cells. By bringing T cells into close proximity with BCMA-expressing myeloma cells, TCEs facilitate the formation of an immunological synapse, leading to T-cell activation and the subsequent release of perforins and granzymes that induce tumor cell lysis. Several agents in this class, such as teclistamab and elranatamab, have received regulatory approval for relapsed or refractory multiple myeloma. Research indicates that soluble BCMA (sBCMA) can act as a decoy receptor, potentially impairing the binding and cytotoxicity of these agents.
