Drug intelligence / Profile preview
Anti-C3d monoclonal antibodies are a class of targeted immunotherapies designed to bind specifically to complement component C3d, a terminal degradation product of complement C3 that remains covalently attached to cell surfaces following complement activation. In B-cell malignancies, complement-fixing monoclonal antibodies (such as rituximab, ofatumumab, or daratumumab) trigger the complement cascade, depositing C3d on tumor cells. While some tumor cells escape killing and lose their primary target antigen (e.g., CD20 or CD38) via trogocytosis, they remain opsonized with C3d. Anti-C3d monoclonal antibodies target this stable neoantigen, providing a 'second hit' to eliminate antigen-loss variants and enhance the efficacy of complement-fixing antibody therapies. Preclinical development of these antibodies has been led by researchers at the National Heart, Lung, and Blood Institute (NHLBI) of the National Institutes of Health (NIH) in collaboration with the University of Virginia and Scripps Research, utilizing clones such as C8xi, 5S8, and d301. Additionally, humanized anti-C3d monoclonal antibody fusion proteins (such as ADX-097/AKB-097, developed by Q32 Bio and Akebia Therapeutics) are being investigated for the treatment of rare kidney diseases by delivering complement regulators directly to sites of tissue inflammation.
