An anti-CAIX monoclonal antibody is a type of targeted biologic designed to bind specifically to carbonic anhydrase IX (CAIX), a cell-surface glycoprotein that is highly overexpressed in clear cell renal cell carcinoma (ccRCC) and various other hypoxic solid tumors, but has very limited expression in normal tissues. By targeting CAIX, these monoclonal antibodies can be utilized for therapeutic purposes—such as inducing antibody-dependent cell-mediated cytotoxicity (ADCC) and complement-dependent cytotoxicity (CDC) against tumor cells—or conjugated to radionuclides (such as zirconium-89 or lutetium-177) for diagnostic immunoPET imaging and targeted radioligand therapy. Prominent examples of anti-CAIX monoclonal antibodies include girentuximab (cG250) and Checkpoint Therapeutics' preclinical candidate CK-303.
