Anti-CD138 antibodies are a class of monoclonal antibodies that target Syndecan-1 (CD138), a transmembrane heparan sulfate proteoglycan that is highly and specifically expressed on the surface of malignant plasma cells in multiple myeloma. CD138 serves as a co-receptor for various growth factors and is involved in cell-cell and cell-matrix interactions, promoting tumor cell survival and proliferation. These antibodies are utilized in several therapeutic formats: as naked antibodies to induce antibody-dependent cellular cytotoxicity (ADCC), as antibody-drug conjugates (ADCs) like indatuximab ravtansine (BT062) to deliver cytotoxic payloads, in radioimmunotherapy (RIT) to deliver alpha or beta-emitting isotopes (e.g., Bismuth-213, Lutetium-177, Iodine-131), and as the targeting component of chimeric antigen receptor (CAR) T-cell therapies. While CD138 is also expressed on some normal epithelial cells, its intense expression on myeloma cells makes it a primary target for immunotherapy in plasma cell dyscrasias.
