Anti-CD19 CAR-Treg is an investigational cell therapy consisting of regulatory T cells (Tregs) engineered to express a chimeric antigen receptor (CAR) specific for the CD19 antigen. Developed by researchers at the IRCCS San Raffaele Scientific Institute, this approach aims to treat B-cell mediated autoimmune diseases, particularly systemic lupus erythematosus (SLE) and lupus nephritis (LN). Unlike conventional CD19-targeted CAR-T therapies that utilize effector T cells to deplete B cells through cytotoxicity, CAR-Tregs are designed to utilize the natural immunosuppressive functions of regulatory T cells. By targeting CD19, these engineered Tregs are redirected to the site of autoreactive B cell activity, where they can suppress B cell proliferation and promote immune tolerance through the secretion of immunomodulatory cytokines, potentially offering a targeted treatment for autoimmunity without the systemic risks of broad immunosuppression.
