Anti-CD19 chimeric antigen receptor peripheral blood lymphocytes (CD19-CAR PBL) is an autologous CAR-T cell therapy developed by the National Cancer Institute (NCI) Surgery Branch. The therapy involves the ex vivo genetic modification of a patient's own peripheral blood lymphocytes using a retroviral vector to express a chimeric antigen receptor (CAR) targeting the CD19 antigen, which is ubiquitously expressed on B-cell malignancies. The specific CAR construct typically utilized in NCI protocols (often referred to as the Kochenderfer CAR) consists of an anti-CD19 single-chain variable fragment (scFv) derived from the FMC63 antibody, linked to a CD28 costimulatory domain and the CD3-zeta signaling chain. Following lymphodepleting conditioning chemotherapy (typically cyclophosphamide and fludarabine), the engineered cells are re-infused into the patient where they recognize and eliminate CD19-positive malignant B-cells. This investigational product served as a foundational technology for the development of commercial CD19-targeted CAR-T therapies.
