Anti-CD19 iCAR-NK cells are a form of genetically engineered natural killer (NK) cell therapy designed to target and eliminate CD19-positive malignant B cells. This approach uses NK cells modified to express an inhibitory chimeric antigen receptor (iCAR) specific for the CD19 antigen. The iCAR module acts as a safeguard, enabling NK cell activity against tumor cells while suppressing off-target or self-reactivity via engineered inhibitory signaling domains. The anti-CD19 targeting enables selective cytolytic activity against CD19-expressing leukemias and lymphomas. The cells can be produced from various sources, including peripheral blood, cord blood, or induced pluripotent stem cells (iPSCs). The engineered iCAR component often incorporates signaling domains from natural inhibitory receptors to counterbalance or regulate excessive NK cell activity, improving safety profiles relative to standard CAR-NK or CAR-T approaches. This platform is experimental and primarily under preclinical and early phase clinical investigation for hematologic malignancies.
