Anti-CD19-TCRz-CD137 CAR-T is a second-generation Chimeric Antigen Receptor (CAR) T-cell therapy. It involves genetically engineering a patient's own T cells (autologous) or donor T cells (allogeneic) to express a CAR. This CAR is designed with an extracellular single-chain variable fragment (scFv) that specifically targets the CD19 protein, which is widely expressed on B-cell malignancies. The intracellular signaling component of the CAR includes the CD3-zeta (TCRz) domain, responsible for initiating primary T-cell activation, and a CD137 (4-1BB) co-stimulatory domain. The inclusion of the CD137 co-stimulatory domain is critical for enhancing T-cell proliferation, cytokine secretion, resistance to apoptosis, and improving the in vivo persistence and overall antileukemic efficacy of the modified T cells. Upon binding to CD19 on cancer cells, these engineered T cells become activated, leading to the recognition and destruction of CD19-expressing malignant B cells. This therapy is primarily developed for the treatment of hematological malignancies such as B-cell acute lymphoblastic leukemia and lymphoma.
