Anti-CD49d antibodies are monoclonal antibodies designed to target the alpha-4 subunit of the VLA-4 (Very Late Antigen-4) integrin, also known as CD49d or ITGA4. CD49d is a critical adhesion molecule expressed on various leukocytes and malignant B-cells, where it mediates binding to VCAM-1 and extracellular matrix proteins. This interaction facilitates the homing and retention of cells within protective microenvironmental niches, such as the bone marrow stroma. In aggressive B-cell malignancies like diffuse large B-cell lymphoma (DLBCL), CD49d-mediated adhesion contributes to immune exclusion and resistance to therapies. By blocking CD49d, these antibodies mobilize malignant cells from sanctuary sites into the peripheral circulation, a process often described as "evicting" the tumor cells. Research presented at EHA 2026 indicates that this mobilization can synergize with T-cell redirecting immunotherapies, such as CD3xCD20 bispecific antibodies, by overcoming niche-mediated resistance and enhancing tumor cell susceptibility to immune-mediated killing.
