Anti-CD6 antibodies are a class of therapeutic monoclonal antibodies that target the CD6 receptor, a co-stimulatory glycoprotein primarily expressed on the surface of T cells and B-1a cells. CD6 plays a critical role in T cell activation, proliferation, and trafficking by interacting with its ligand, Activated Leukocyte Cell Adhesion Molecule (ALCAM/CD166). By modulating or blocking the CD6-ALCAM signaling pathway, these antibodies aim to treat autoimmune and inflammatory diseases by reducing the infiltration of pathogenic T cells into tissues without causing broad T cell depletion. The most clinically advanced anti-CD6 antibody is itolizumab (EQ001), a humanized IgG1 monoclonal antibody originally developed by Biocon and currently being developed by Equillium. Itolizumab is approved in India for the treatment of chronic plaque psoriasis and is undergoing global clinical evaluation for conditions such as acute graft-versus-host disease (aGVHD), lupus nephritis, and systemic lupus erythematosus (SLE). Preclinical research has demonstrated that anti-CD6 modulation can significantly ameliorate end-organ damage, including skin and kidney disease, in murine models of lupus.
