An anti-CD71 antibody-drug conjugate (ADC) is a targeted cancer therapeutic designed to deliver a cytotoxic payload directly to cells expressing CD71 (transferrin receptor 1, TfR1). CD71 is highly overexpressed in many malignant tissues, including solid tumors and hematological malignancies, due to the high iron demands of rapidly proliferating cancer cells. However, because CD71 is also expressed on normal proliferating tissues, such as hematopoietic progenitor cells, traditional anti-CD71 ADCs have historically faced severe on-target, off-tumor toxicities, such as life-threatening myelosuppression. To overcome this narrow therapeutic window, developers have explored conditionally activated formats, such as Probody drug conjugates (PDCs) like CX-2029 (developed by CytomX Therapeutics in collaboration with AbbVie), which remain masked in circulation and are selectively activated by proteases in the tumor microenvironment. Other clinical-stage candidates include INA03 (developed by Inatherys), a humanized anti-CD71 ADC carrying a monomethyl auristatin E (MMAE) payload, which has been evaluated in clinical trials for relapsed or refractory acute leukemias.
