Anti-CGRP antibodies are a class of monoclonal antibodies that target the calcitonin gene-related peptide (CGRP) pathway, which plays a central role in the transmission of pain and the development of neurogenic inflammation. These biologics either bind to the CGRP ligand itself or block the CGRP receptor, effectively inhibiting the vasodilatory and pro-inflammatory effects of the neuropeptide. Originally developed and approved for the prevention of migraine (e.g., erenumab, fremanezumab, galcanezumab, and eptinezumab), these antibodies are now being investigated for other conditions characterized by headache and elevated intracranial pressure. A notable clinical trial sponsored by Rigshospitalet is evaluating the efficacy of anti-CGRP antibody treatment in preventing chronic headaches in patients with idiopathic intracranial hypertension (IIH).
