Overview

Anti-GARP CAR T cell therapy is an investigational chimeric antigen receptor (CAR) T-cell therapy targeting glycoprotein-A repetitions predominant (GARP), also known as leucine-rich repeat-containing protein 32 (LRRC32). GARP is a transmembrane protein that plays a critical role in the activation and surface expression of transforming growth factor-beta (TGF-β), a potent immunosuppressive cytokine. GARP is highly expressed on the surface of regulatory T cells (Tregs) and various malignant cells, including high-grade gliomas, where it contributes to a suppressed immune environment. By engineering a patient's T cells to express a CAR specific to GARP, the therapy aims to achieve dual activity: the direct cytotoxic elimination of GARP-expressing tumor cells and the depletion of immunosuppressive Tregs within the tumor microenvironment. This therapy is currently being evaluated in clinical trials for recurrent malignant gliomas, where it is administered via an intracavitary route to bypass the blood-brain barrier and target the tumor site directly.
