The anti-HSPA5 antibody-drug conjugate is an experimental therapeutic designed to target Heat Shock Protein A5 (HSPA5), also known as GRP78, specifically on the surface of cancer cells. While HSPA5 is normally localized within the endoplasmic reticulum (ER) as a molecular chaperone, radiation therapy (RT) triggers its translocation to the cell surface in gastric and colorectal cancer cells. This translocation is a cancer-specific mechanism involving O-glycosylation and binding to N-acetylgalactosaminyltransferase 2 (GALNT2). The ADC exploits this radiation-induced vulnerability to deliver a cytotoxic payload directly to tumor cells, particularly targeting tumorigenic populations enriched for stemness and EMT markers like CD44 and N-cadherin. By requiring radiation to expose the target on the cell surface, the ADC aims to achieve high anti-tumor specificity and minimize on-target off-tumor toxicity in healthy tissues where HSPA5 remains sequestered in the ER.
