An anti-ICAM antibody (specifically targeting Intercellular Adhesion Molecule 1, or ICAM-1/CD54) is a monoclonal antibody designed to inhibit the interaction between ICAM-1 on endothelial cells and its ligands (such as LFA-1) on leukocytes. This blockade prevents leukocyte adhesion and transendothelial migration, thereby reducing inflammation and tissue damage in conditions like ischemia-reperfusion injury, stroke, and autoimmune diseases. Several anti-ICAM-1 antibodies, such as enlimomab and bersanlimab, have been investigated in clinical trials for indications including acute ischemic stroke, rheumatoid arthritis, renal transplantation, and multiple myeloma, though clinical efficacy has been limited.
