Anti-IGF-1R monoclonal antibodies are a class of biologic therapeutics designed to target and inhibit the insulin-like growth factor 1 receptor (IGF-1R). In the context of thyroid eye disease (TED), also known as Graves' ophthalmopathy, IGF-1R is significantly overexpressed on orbital fibroblasts and forms a functional signaling complex with the thyroid-stimulating hormone receptor (TSHR). By blocking IGF-1R, these antibodies inhibit the downstream signaling pathways that drive the inflammatory response, the production of glycosaminoglycans (such as hyaluronic acid), and the differentiation of fibroblasts into adipocytes. This mechanism effectively reduces orbital tissue expansion, proptosis, and diplopia. The "Thyroid Eye Disease Specialty Cohort Project" (ChiCTR2600129274) is a prospective observational study sponsored by Shanghai Ninth People's Hospital and Shanghai Jiao Tong University that utilizes clinical, imaging, and multi-omics data to evaluate treatment responses to these targeted biologics and glucocorticoids in patients with TED.
