Overview

anti-IGHV3-23 CAR T cells (also known as CART3-23) represent a novel chimeric antigen receptor T-cell (CAR T) immunotherapy designed to target B-cell malignancies that express the IGHV3-23 gene segment on their B-cell receptors. This therapy aims to overcome limitations of CD19-directed CAR T cells, such as antigen-negative escape and long-term B-cell aplasia leading to increased infection risk. By specifically targeting IGHV3-23, which is expressed by less than 15% of normal B cells, this CAR T cell therapy is hypothesized to efficiently eliminate malignant cells while sparing the majority of healthy B cells, thereby potentially reducing immunosuppression and improving the safety profile. The CAR construct, designated CD8-41BB-CD3z, utilizes a single-chain variable fragment (scFv) derived from the 3C9 monoclonal antibody to recognize the IGHV3-23+ B-cell receptor.
