Anti-IL-22 monoclonal antibodies are biologic therapeutics designed to target and neutralize the cytokine interleukin-22 (IL-22). IL-22 is a member of the IL-10 family that plays a critical role in skin homeostasis and inflammation by acting on epithelial cells. By binding to IL-22, these antibodies prevent the cytokine from interacting with its heterodimeric receptor complex (comprising IL-22R1 and IL-10R2), thereby inhibiting downstream signaling pathways such as STAT3. This blockade reduces pro-inflammatory responses, epidermal hyperplasia, and the production of antimicrobial peptides. The most prominent clinical candidate in this class is fezakinumab (formerly ILV-094), which has been evaluated in Phase 2 trials for moderate-to-severe atopic dermatitis and psoriasis. Clinical data for fezakinumab has shown significant improvements in disease scores, particularly in patients with high baseline IL-22 expression, with sustained responses observed even after the cessation of treatment.
