An anti-IL-23R antibody is a monoclonal antibody designed to specifically bind to the interleukin-23 receptor (IL-23R), thereby blocking the signaling pathway of the pro-inflammatory cytokine IL-23. IL-23 is a critical driver for the differentiation, maintenance, and proliferation of Th17 cells, particularly tissue-resident memory Th17 (TRM17) cells, which are central to the pathogenesis of chronic inflammatory diseases such as psoriasis and inflammatory bowel disease (IBD). By targeting the receptor rather than the IL-23 ligand itself, these antibodies prevent the assembly of the functional IL-23 receptor complex and inhibit downstream STAT3 activation. While several approved biologics target the IL-23 ligand (p19 subunit) or the shared p40 subunit of IL-12/23, antibodies specifically targeting the IL-23 receptor are primarily utilized in preclinical research to study chronic skin inflammation and host defense mechanisms, or are in early stages of clinical development.
