Anti-ILT3 CAR-T cells are an investigational chimeric antigen receptor (CAR) T-cell therapy designed to target Immunoglobulin-like transcript 3 (ILT3), also known as LILRB4 or CD85k. Developed by Zhejiang Provincial People's Hospital, this therapy is primarily aimed at treating monocytic subtypes of acute myeloid leukemia (AML), specifically FAB M4 and M5, where ILT3 is highly and selectively expressed on the surface of malignant blasts. ILT3 is an inhibitory immune checkpoint molecule that contributes to immune evasion by suppressing T-cell activity and promoting an immunosuppressive microenvironment. By engineering T cells to express a CAR specific for ILT3, the therapy enables the direct recognition and cytotoxic elimination of leukemic cells while potentially reversing ILT3-mediated immunosuppression. It is currently being evaluated in early-phase clinical trials for patients with relapsed or refractory AML.
