Anti-integrin alpha L antibodies are a class of monoclonal antibodies that target the alpha L subunit (CD11a) of the leukocyte function-associated antigen-1 (LFA-1) integrin complex. LFA-1 is a heterodimeric protein (αLβ2) expressed on the surface of leukocytes that plays a critical role in cell-cell adhesion and signaling by binding to intercellular adhesion molecules (ICAMs). In the context of T-cell acute lymphoblastic leukemia (T-ALL), research has demonstrated that integrin alpha L signaling is essential for the survival of leukemic blasts by facilitating supportive interactions with tumor-associated myeloid cells within the bone marrow microenvironment. Blockade of this pathway using anti-integrin alpha L antibodies has been shown to disrupt these protective contacts, leading to reduced leukemic burden and improved survival in preclinical models. Historically, this class included efalizumab, which was used for psoriasis before being withdrawn due to safety concerns.
