Overview

An anti-integrin $\alpha$v$\beta$1 small molecule is a targeted therapeutic agent designed to inhibit the $\alpha$v$\beta$1 integrin heterodimer, a cell-surface receptor primarily expressed on fibroblasts. This integrin plays a critical role in tissue fibrogenesis by mediating fibroblast-extracellular matrix (ECM) interactions and facilitating the activation of latent TGF-$\beta$. In the context of inflammatory bowel diseases (IBD), such as ulcerative colitis (UC), $\alpha$v$\beta$1 is significantly upregulated in inflamed and fibrotic tissue layers. Small molecule inhibitors of $\alpha$v$\beta$1 aim to disrupt these pro-fibrotic signaling pathways, thereby promoting the resolution of fibrosis and reducing tissue stiffness. Preclinical research, including studies presented at ECCO 2026 involving collaborations between the Cleveland Clinic, Pfizer, and Eli Lilly, has demonstrated that targeting $\alpha$v$\beta$1 with small molecules can lead to fibrosis resolution in both in vitro and in vivo models of transmural UC.
