Overview

Anti-integrin beta7 refers to a class of monoclonal antibodies designed to target the $\beta7$ subunit of integrin receptors. This subunit is a component of two distinct heterodimers: $\alpha4\beta7$ and $\alpha E\beta7$. The $\alpha4\beta7$ integrin is primarily responsible for the trafficking of T-lymphocytes to the intestinal mucosa through its interaction with mucosal vascular addressin cell adhesion molecule-1 (MAdCAM-1). The $\alpha E\beta7$ integrin facilitates the retention of these lymphocytes within the intestinal epithelium by binding to E-cadherin. By antagonizing the $\beta7$ subunit, these therapeutic agents provide a dual mechanism of action that inhibits both the entry and the persistence of inflammatory cells in the gut. This approach is primarily investigated for the treatment of inflammatory bowel diseases (IBD), such as ulcerative colitis and Crohn's disease, aiming to provide gut-selective immunosuppression. Etrolizumab, developed by Roche and Genentech, is the most prominent clinical candidate in this category.
