Overview

This is an autologous chimeric antigen receptor (CAR) T-cell therapy targeting the interleukin-13 receptor alpha 2 (IL-13Rα2) antigen. IL-13Rα2 is a tumor-associated antigen frequently overexpressed in pediatric high-grade gliomas, including diffuse intrinsic pontine glioma (DIPG), while showing minimal expression in normal brain tissue. The CAR construct utilized in this program, developed by Fundació Sant Joan de Déu, incorporates a modified IL-13 ligand (specifically the E13Y mutation) to enhance binding affinity for IL-13Rα2 while reducing cross-reactivity with the more widely expressed IL-13Rα1. The second-generation CAR also includes 4-1BB (CD137) and CD3-zeta signaling domains to promote T-cell activation, persistence, and cytotoxicity. The therapy is typically administered locoregionally via intracerebroventricular injection to bypass the blood-brain barrier and maximize exposure to the tumor site in the brainstem.
