Anti-mesothelin CAR-NK cells are a type of engineered cellular immunotherapy in which natural killer (NK) cells are modified ex vivo to express a chimeric antigen receptor (CAR) that specifically targets mesothelin, a cell surface antigen commonly overexpressed in several solid tumors such as ovarian cancer, mesothelioma, pancreatic cancer, and some lung cancers. The typical CAR construct includes an extracellular single-chain variable fragment (scFv) recognizing mesothelin, a transmembrane domain, co-stimulatory signaling domains such as 2B4, and a CD3ζ signaling domain. These enhancements potentiate NK cell cytotoxicity against tumor cells expressing mesothelin. Anti-mesothelin CAR-NK cells may be derived from various sources, including umbilical cord blood, induced pluripotent stem cells (iPSCs), or NK cell lines. Preclinical studies have shown potent anti-tumor activity in models of ovarian cancer with low toxicity, and clinical trials are ongoing to assess safety and efficacy in humans[7].
