Anti-mesothelin chimeric antigen receptor cytokine-induced memory-like natural killer cells (anti-MSLN CAR CIML NK cells) is an investigational adoptive cell immunotherapy for the treatment of mesothelin-expressing solid tumors and hematological malignancies. The therapy utilizes primary natural killer (NK) cells that are briefly activated ex vivo with a cytokine cocktail of IL-12, IL-15, and IL-18 to induce a 'memory-like' (CIML) phenotype, which enhances effector function, metabolic fitness, and persistence. These CIML NK cells are then genetically engineered using a lentiviral system to express a chimeric antigen receptor (CAR) targeting the proximal domain of mesothelin (MSLN), incorporating 4-1BB and CD3ζ signaling domains. This approach combines the innate anti-tumor capabilities and safety profile of NK cells with the high specificity of CAR-mediated targeting. Preclinical studies have demonstrated significant cytotoxicity against ovarian cancer cell lines and patient-derived organoids, as well as tumor growth inhibition in xenograft models.
