The anti-pTα antibody-drug conjugate (ADC) is an experimental targeted immunotherapy designed for the treatment of T-cell acute lymphoblastic leukemia (T-ALL). It consists of a monoclonal antibody specific to the invariant pTα subunit of the human pre-T-cell receptor (pre-TCR) conjugated to a cytotoxic payload. The pre-TCR is a critical signaling complex expressed during early T-cell development and has been identified as a specific biomarker for leukemia-initiating cells (LICs) in T-ALL. By targeting pTα, which is expressed on leukemic blasts but has highly restricted expression in normal tissues, the ADC selectively eliminates LICs while sparing mature T cells. Preclinical studies in patient-derived xenograft (PDX) models have demonstrated that anti-pTα ADC treatment significantly inhibits LIC activity and tumor progression, offering a potential therapeutic strategy for patients with relapsed or refractory T-ALL.
