Anti-SARS-CoV-2 equine hyperimmune serum is a passive immunotherapy developed by the **Caja Costarricense de Seguro Social (CCSS)** in collaboration with the **Instituto Clodomiro Picado (ICP)** of the University of Costa Rica. The therapy consists of purified F(ab')2 fragments of polyclonal antibodies obtained from horses immunized with recombinant SARS-CoV-2 proteins. Two formulations were initially developed: the 'S-type,' which targets the S1 subunit of the viral spike protein, and the 'M-type,' which targets a combination of viral proteins including S1, nucleocapsid (N), envelope (E), and membrane (M). The product is designed to neutralize the virus and prevent its entry into host cells, thereby reducing viral load and preventing the progression of COVID-19 in hospitalized patients with moderate to severe disease. Clinical trials, such as SECR-02, have specifically evaluated the efficacy and safety of different doses of the S-type formulation as an adjunct to standard care.
