Anti-SEM4A CAR T cells are an experimental chimeric antigen receptor (CAR) T-cell therapy designed to target Semaphorin 4A (SEM4A), a protein identified as a potential therapeutic target in multiple myeloma. Developed by researchers at the University of Ostrava, these CAR T cells utilize a single-chain variable fragment (scFv) specific for SEM4A fused to 4-1BB and CD3ζ signaling domains. Preclinical characterization has demonstrated specific cytotoxicity against SEM4A-positive myeloma cell lines and significant production of pro-inflammatory cytokines such as IFN-γ and IL-2 upon antigen recognition. A significant challenge identified in early studies is the expression of SEM4A on activated T cells, which leads to early fratricide of SEM4A+ CD8+ T cells and a shift toward an effector memory phenotype, potentially impacting the persistence and long-term efficacy of the product.
