Anti-third party cytolytic T-lymphocytes (CTLs) are an investigational adoptive cell therapy used in the context of allogeneic hematopoietic stem cell transplantation (HSCT). These donor-derived T-cells are sensitized and expanded *ex vivo* against antigens from an unrelated third-party donor. The primary goal of this strategy is to generate a population of "veto cells" or regulatory cells that can selectively deplete or suppress the host's alloreactive T-cells. This selective immunosuppression is designed to promote donor cell engraftment and reduce the incidence of graft-versus-host disease (GVHD) without inducing broad, systemic immune suppression, thereby potentially maintaining beneficial graft-versus-leukemia (GVL) effects. This therapy has been primarily investigated for use in patients with hematologic malignancies such as leukemia, lymphoma, and myeloma.
