Overview

This is an experimental multi-agent immunosuppressive regimen that combines anti-thymocyte globulin (ATG), cyclophosphamide, sirolimus, abatacept, and alemtuzumab to achieve profound T‑cell depletion and functional suppression, primarily in the context of allogeneic hematopoietic stem cell transplantation or other intense immunomodulatory settings. ATG and alemtuzumab are biologic T‑cell–depleting antibodies (polyclonal anti‑T‑cell immunoglobulin and anti‑CD52 monoclonal antibody, respectively) that reduce circulating lymphocytes and modulate T‑cell subsets.[3][2] Cyclophosphamide, typically used at high doses shortly after transplantation (post‑transplant cyclophosphamide, PTCy), is an alkylating agent that preferentially eliminates highly proliferating alloreactive T cells, thereby preventing graft‑versus‑host disease (GvHD) and graft rejection.[3][5][11] Sirolimus is an mTOR‑inhibiting small molecule that blocks IL‑2–driven T‑cell proliferation and promotes regulatory T‑cell expansion, and has been combined with PTCy as a calcineurin‑inhibitor–free GvHD prophylaxis platform.[1][5] Abatacept is a CTLA‑4–Ig fusion protein that blocks CD28‑mediated co‑stimulation by binding CD80/CD86 on antigen‑presenting cells, attenuating T‑cell activation and reducing acute GvHD in allogeneic transplantation.[4] Together, this intensive combination is designed to synergistically suppress and reprogram donor and host T‑cell responses to minimize GvHD and rejection while allowing engraftment and, in malignancy, preserving graft‑versus‑leukemia effects.[3][4][5]
