An anti-TLR9 antibody is a therapeutic agent designed to target and inhibit the activity of Toll-like receptor 9 (TLR9). While TLR9 is traditionally recognized as an endosomal receptor for hypomethylated CpG DNA in immune cells, recent research has identified its functional expression on the surface of platelets, particularly in the context of sickle cell disease (SCD). In SCD, elevated levels of circulating cell-free DNA and neutrophil extracellular traps (NETs) activate platelet TLR9, triggering thrombo-inflammatory pathways that lead to the formation of neutrophil-platelet aggregates. This process is a key driver of lung vaso-occlusion and the development of acute chest syndrome (ACS). By blocking TLR9, these antibodies aim to reduce platelet activation and prevent sterile lung injury and vaso-occlusive crises.
