Overview

This is a combination therapy consisting of an anti-tumor necrosis factor alpha (anti-TNFα) biologic agent and a thiopurine immunosuppressant. Anti-TNFα agents are monoclonal antibodies that target and neutralize tumor necrosis factor alpha (TNF-α), a key cytokine in inflammatory processes. Commonly used anti-TNFα drugs include infliximab, adalimumab, and certolizumab pegol. Thiopurines—such as azathioprine (AZA), 6-mercaptopurine (6-MP), or 6-thioguanine—are small molecule immunosuppressants that inhibit purine synthesis, leading to reduced lymphocyte proliferation. The primary indication for this combination is the induction and maintenance of remission in moderate to severe inflammatory bowel disease (IBD), including Crohn’s disease and ulcerative colitis. The rationale for combining these drugs is to enhance efficacy by reducing the formation of antibodies against the biologic agent (thus improving its pharmacokinetics) while providing additional immunosuppression from the thiopurine[2][3][4]. This approach has been shown in clinical trials such as SONIC and UC SUCCESS to be superior to monotherapy with either drug alone for inducing remission[2][4]. However, combined use increases risks of infections and malignancies compared with monotherapy.
