anti-TNF + ustekinumab

This is a combination therapy consisting of an anti-tumor necrosis factor (anti-TNF) agent and ustekinumab, used primarily in the management of refractory inflammatory bowel disease (IBD), including Crohn’s disease and ulcerative colitis. Anti-TNF agents are monoclonal antibodies or fusion proteins that neutralize tumor necrosis factor alpha (TNF-α), a key pro-inflammatory cytokine involved in autoimmune diseases. Ustekinumab is a fully human monoclonal antibody targeting the p40 subunit shared by interleukin-12 (IL-12) and interleukin-23 (IL-23), thereby inhibiting their activity. The rationale for combining these drugs is to achieve complementary inhibition of distinct inflammatory pathways—TNF-mediated and IL-12/23-mediated—potentially improving clinical response rates in patients who have failed monotherapy with either agent. Clinical studies suggest this combination can be effective for inducing remission in difficult-to-treat IBD cases, with pooled clinical response rates as high as 81–92% and remission rates up to 64%[1][5]. Safety data indicate an acceptable adverse event profile when used together[5].
