Overview

Anti-transferrin receptor antibodies are a category of monoclonal antibodies that target the transferrin receptor 1 (TfR1, also known as CD71), a protein highly expressed on the surface of proliferating cells and brain endothelial cells. These antibodies are developed for two primary therapeutic applications. In hematology and oncology, they are used to bind TfR1 and inhibit the influx of iron into rapidly dividing cells, such as erythroid precursors in polycythemia vera or malignant cells in aggressive NK leukemia, thereby suppressing cell growth and differentiation. A leading candidate in this space is PPMX-T003, a fully human monoclonal antibody developed by Perseus Proteomics, which has demonstrated efficacy in Phase 1 trials by controlling hematocrit levels and reducing phlebotomy requirements. In neurology, anti-TfR antibodies are utilized as "shuttles" for receptor-mediated transcytosis, facilitating the delivery of therapeutic agents (e.g., multispecific antibodies, enzymes, or small molecules) across the blood-brain barrier (BBB). Companies like Adimab have developed panels of these antibodies, including single-domain antibodies (sdAbs), to optimize BBB permeability for various neurological indications.
