The regimen consists of intensive androgen ablation therapy administered after definitive radiation therapy, with or without the addition of the PARP inhibitor niraparib. The androgen ablation component is defined as either abiraterone acetate plus prednisone combined with androgen deprivation therapy (ADT), or apalutamide combined with ADT. This therapeutic approach aims to achieve maximal suppression of the androgen signaling axis while simultaneously inhibiting DNA repair mechanisms to improve clinical outcomes in biomarker-selected patients.
