This is a combination therapy of antiepileptic drugs (AEDs) and sirolimus, primarily used in the management of epilepsy associated with tuberous sclerosis complex (TSC), especially in pediatric populations. Antiepileptic drugs are a diverse group of small molecules that act via various mechanisms to reduce neuronal excitability and prevent seizures, including modulation of sodium channels, enhancement of GABAergic inhibition, inhibition of glutamatergic transmission, and other pathways. Sirolimus is an mTOR inhibitor that targets the mammalian target of rapamycin (mTOR) pathway; it reduces abnormal cell growth and proliferation seen in TSC by inhibiting mTOR activity. In TSC-related epilepsy, sirolimus may improve neuronal function, decrease neuroinflammation, lower neural excitability, and provide neuroprotection—thereby reducing seizure frequency and severity when combined with standard AEDs[1][4][7]. This combination is under clinical investigation for its efficacy in refractory or syndromic epilepsies where mTOR pathway dysregulation plays a role.
