An antiPCSK9 vaccine is an investigational therapeutic vaccine that stimulates the immune system to generate antibodies targeting proprotein convertase subtilisin/kexin type 9 (PCSK9), a protein that plays a key role in cholesterol metabolism. By inducing anti-PCSK9 antibodies, these vaccines inhibit PCSK9's interaction with the low-density lipoprotein receptor (LDL-R), leading to increased clearance of LDL cholesterol (LDL-C) from the bloodstream. Various vaccine platforms have been explored, including peptide-based vaccines conjugated to carrier proteins and virus-like particle (VLP)-based constructs. Clinical development has included candidates such as AT04A, AT06A (by AFFiRiS), VXX-401, and a bivalent VLP vaccine targeting two PCSK9 epitopes. The approach aims to provide a long-lasting, potentially cost-effective alternative to monoclonal antibody-based PCSK9 inhibitors for the treatment of hypercholesterolemia and atherosclerotic cardiovascular disease. As of now, antiPCSK9 vaccines are in early to mid-stage clinical development, with no product approved for clinical use[1][3][4][8].
