Antipyrine is a synthetic analgesic and antipyretic drug that has been used since the late 19th century. It is primarily used for the symptomatic treatment of acute otitis media (middle ear infection), most commonly in combination with benzocaine as an otic (ear) solution to relieve ear pain and swelling. Antipyrine can also be administered orally or as ear drops and has historically been used to test hepatic drug-metabolizing enzyme activity. Its mechanism of action involves inhibition of cyclooxygenase enzymes (COX-1 and COX-2), leading to reduced synthesis of prostaglandins involved in pain and fever signaling[2][5][8]. While it was once widely used as an oral antipyretic/analgesic agent for various indications including fever and mild pain relief[5], its use today is largely limited to topical otologic applications.
